Cardiopulmonary and inflammatory biomarkers in dog and cat heartworm disease

Један од најзначајнијих кардиопулмоналних и инфламаторних биомаркера код дирофилариозе паса и мачака је Д-димер, производ разградње фибрина детектабилан тек након што се угрушак разгради. Због тога може да послужи за валидну потврду дијагнозе плућне тромбоемболије проузроковане нематодом Dirofilaria immitis. Његове концентрације се повећавају са напредовањем тежине болести, као и током третмана адултицидима што се показало као вредан параметар у раној дијагностици и праћењу хроничног тока болести. Испитивањем осетљивости тропонина I, миоглобина и натриуретичних пептида (NT-proBNP) на присуство повреде миокарда и срчане инсуфицијенције код хроничних инфекција врстом D. immitis, установљено је благо побољшање код пацијената након третмана одраслих облика паразита. Код паса инфицираних са D. immitis током акутне фазе инфекције активирају се протеини акутне фазе (АПП), посебно Ц-реактивни протеин (ЦРП) чије повећање је у корелацији са развојем инфламаторних процеса, напредовањем болести и ...појавом плућне хипертензије (ПХ) која настаје током раста паразита у пулмоналним артеријама. Једина објављена студија о деловању биомаркера код мачака серопозитивних на D. immitis, извештава о значајном повећању концентрације АПП: Серумског амилоида А, Хаптоглобина и Церулоплазмина. У току 2021. године на Факултету ветеринарске медицине Универзитета у Београду, на Клиници за мале животиње, 30 паса различитих старосних категорија позитивних на D. immitis тестирано је на тропонин I и натриуретични пептид у различитим фазама лечења, са циљем да се изврши процена степена повреде миокарда и срчане инсуфицијенције. Утврђена је веома значајна разлика (p<0.001) између просечних вредности Nt-proBNP (pmol/L) код паса са кардиомиопатијом (727.5±126.4) у односу на псе са дирофилариозом (420.3±923.9). Просечне вредности cTnI (ng/ml) су биле ниже код паса до шест година старости у односу на старије псе. Утврђена је значајна разлика (p<0.05) у вредности cTnI (ng/ml) код паса са кардиомиопатијом у односу на старосну категорију (0.66±0.17). Добијени резултати имају посебан клинички значај за поуздано регистровање и процену кардиопулмоналних повреда и инфламаторне активности током акутне и хроничне фазе развоја дирофилариозе месоједа.
One of the most important cardiopulmonary and inflammatory biomarkers in canine and feline heartworm disease is D-dimer, a fibrin degradation product detectable only after the clot has degraded. Therefore, it can serve as a valid confirmation of the diagnosis of pulmonary thromboembolism caused by the nematode D. immitis. Its concentrations increase with the progression of the severity of the disease, as well as during treatment with adulticides, which proved to be a valuable parameter in the early diagnosis and monitoring of the chronic course of the disease. Examination of the susceptibility of troponin I, myoglobin, and natriuretic peptides (NT-proBNP) to the presence of myocardial injury and heart failure in chronic D. immitis infections revealed a slight improvement in patients after treatment with adult parasites. A very significant difference (p 0.001) was found between the average values of Nt-proBNP (pmol/L) in dogs with cardiomyopathy (727.5 ± 126.4) compared to dogs with heartworm disease (420.3 ± 923.9). Mean cTnI values (ng/ml) were lower in dogs up to six years of age compared to older dogs. A significant difference (p <0.05) in the value of cTnI (ng/ml) was found in dogs with cardiomyopathy concerning the age category (0.66 ± 0.17). The obtained results have a special clinical significance for the reliable registration and assessment of cardiopulmonary injuries and inflammatory activity during the acute and chronic phase of the development of carnivorous heartworm disease.
